JFCS East Bay is proud to partner with Resources for Community Development (RCD) to provide on-site care coordination and social work for low-income seniors at Villa Vasconcellos in Walnut Creek and Berrellesa Palms in Martinez.
At each facility, our geriatric social workers help residents access the services they need, communicate with families, provide aging advice and planning, act as a liaison between the property managers and residents, and plan social gatherings and community events. These services help vulnerable seniors continue living independently for as long as possible.
